Intake Visit
75–90 minutes — full history, exam, and treatment plan.
Regular Visit
30 minutes — assessment, adjustment, and muscle work.
Extended Visit
45 minutes — added modalities and deeper care.
Re-Evaluation
45 minutes — reassessing progress and your plan.
Intake — $150
Your first visit is an extremely comprehensive intake session, running 75–90 minutes. We take the time to understand your history, your unique needs, and your concerns, and conduct a detail-oriented exam to build a treatment plan tailored to your specific condition. The $150 fee covers this full assessment, plus an adjustment and muscle work that same day if indicated.
Regular visits — $70
Once your intake is complete, regular visits are $70 for a focused 30-minute session. Each visit includes a detailed check of your posture, joint restrictions, and muscle tone, plus the adjustments and treatments you need that day — working toward less discomfort, better mobility, and overall wellness.
Extended visits — $90
Some days call for more time and attention than a regular visit — extended visits are $90 for 45 minutes and include a more thorough exam, in-depth adjustments, and supportive modalities like heat therapy, cold therapy, or muscle relaxation techniques for specific conditions, activities, or personal preferences.
Re-evaluation visits — $90
Your health changes over time, so periodic 45-minute re-evaluation visits let us assess your progress and adjust your treatment plan as needed — an essential part of your ongoing care.
Pre-payment discount
We offer a 10% discount on all 10-visit treatment packages, as a way to help you save on ongoing care. This discount doesn't apply to intake or re-evaluation appointments, and pre-paid packages must be paid by cash or check — for checks, the discount applies once the check has cleared.
Why we're out-of-network with insurance
Golden Bear Chiropractic is out-of-network with all insurance and non-participating with Medicare — on purpose. Accepting insurance would require itemizing every service, which raises costs for both the practice and patients, especially those in marginalized communities who may otherwise be priced out of care. Staying out-of-network is the only way we can keep our pricing inclusive and affordable for everyone.
Getting a receipt for reimbursement
Though we don't bill insurance directly, we'll provide a detailed superbill on request — including our EIN, Dr. Corey's NPI, your CPT codes, and the associated ICD-10 codes — for you to submit for reimbursement yourself. Please allow up to 3 weeks after your visit for this. We do our best to avoid denials, but every plan is different; if your insurer denies a claim, it's on you to get the reason for denial and bring it to your next visit. No one at Golden Bear will speak with your insurance company without a signed Health Information Release Form — outside of the superbill itself, insurance matters are your responsibility.
